    Environmental Health: Assessing Mitigating Risk in Public Health

    CHAPTER ONE

    1.1 Introduction

    Public Health is the science and art of preventing disease, prolonging life, and promoting health through organized community efforts. This includes a wide range of activities such as health education, disease prevention, health policy development, and the regulation of health services (World Health Organization, 2021). The intersection of public health and environmental health is a critical area of study, particularly when considering the multifaceted risks posed by environmental factors. Risk mitigation in this context involves the identification, assessment, and prioritization of risks followed by the coordinated application of resources to minimize, monitor, and control the probability and impact of unfortunate events.

    Effective risk mitigation strategies are essential to safeguard public health against the adverse effects of environmental hazards such as pollution, climate change, and exposure to toxic substances. Environmental health encompasses a wide range of factors including air and water quality, chemical exposures, and the built environment, all of which can significantly influence public health outcomes. According to the World Health Organization (WHO), approximately 24% of the global disease burden and 23% of all deaths can be attributed to environmental factors (WHO, 2016). Therefore, a robust approach to risk mitigation is imperative.

    As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the Aim and Objectives of the study. Others are Significance of the study, Scope of work, Research hypothesis and questions, Limitation of the study and Definition of terms.


    1.2 Background of Study

    The historical evolution of assessing risk mitigation in public health for environmental health reflects a growing understanding of the critical links between the environment and human health. From early sanitation measures to modern regulatory frameworks and technological advancements, the field has continually adapted to address emerging challenges and protect public health. Environmental health, a crucial component of public health, focuses on the interrelationship between people and their environment, promoting human health and well-being, and fostering healthy and safe communities. In recent years, the importance of effective risk mitigation strategies in environmental health has become increasingly apparent, given the escalating threats posed by climate change, pollution, and emerging infectious diseases.

    The late 20th and early 21st centuries have seen a growing recognition of the complex interplay between environmental factors and public health. Modern risk assessment methodologies have become more sophisticated, incorporating advances in epidemiology, toxicology, and environmental science. Tools such as Geographic Information Systems (GIS) and remote sensing technology have enhanced the ability to monitor environmental hazards and predict health impacts. The adoption of the precautionary principle has also been significant, advocating for proactive measures in the face of scientific uncertainty to protect public health. This principle has been instrumental in guiding policies on emerging risks such as endocrine disruptors and nanomaterials (Rogers, 2001).

    The World Health Organization (WHO) defines environmental health as encompassing those aspects of human health, including quality of life, determined by physical, chemical, biological, social, and psychosocial factors in the environment. It involves the assessment and control of these environmental factors that can potentially affect health. Proper management of environmental health is vital to prevent diseases and create health-supportive environments. Environmental health faces numerous challenges such as air and water pollution, hazardous waste, and climate change. Air pollution, for instance, is responsible for millions of premature deaths annually. Water contamination affects drinking water quality and can lead to outbreaks of waterborne diseases. Climate change exacerbates these problems by increasing the frequency and severity of extreme weather events, which can lead to health crises such as heatwaves, floods, and the spread of infectious diseases.

    Risk mitigation in environmental health involves strategies and actions targeted at reducing or eliminating the adverse effects of environmental hazards. This includes regulatory measures, public education, technological advancements, and community-based initiatives. For example, implementing stringent air quality standards, promoting the use of clean energy, and enhancing disaster preparedness can significantly mitigate risks. Therefore, in Nigeria where the research was carried out, the activities that was conducted is to assess the effectiveness of various risk mitigation strategies in the context of public health, specifically targeting environmental health threats.


    1.3 Statement of Problems

    Investigation revealed that one of the primary challenges in assessing risk mitigation for environmental health is the lack of comprehensive data and scientific uncertainty. Limited data availability hampers accurate risk assessments and makes it challenging to quantify the magnitude of potential health impacts. Additionally, scientific uncertainty regarding the long-term effects of environmental exposures complicates risk management decisions.

    Moreover, navigating the regulatory landscape governing environmental health can be complex, with overlapping jurisdictions and fragmented oversight responsibilities. Inadequate enforcement of environmental regulations further undermines risk mitigation efforts, allowing polluters to operate with impunity and exposing communities to health hazards. Strengthening regulatory frameworks and enhancing enforcement mechanisms are critical for protecting public health.

    Furthermore, assessing risk mitigation in public health for environmental health requires collaboration across multiple disciplines, including epidemiology, toxicology, environmental science, and public policy. However, interdisciplinary coordination can be challenging due to differences in methodologies, terminology, and priorities among various stakeholders. Overcoming these barriers is essential for developing holistic risk management strategies. Hence, it is against this backdrop that this study aims to assess the risk mitigation in public health for environmental health.


    1.4 Aim and Objectives of Study

    The aim of the study is to assess the risk mitigation in public health for environmental health. In achieving this aim, the following specific objectives were laid out as follows:

    1. To analyze the sources and distribution of environmental health risks, including natural and anthropogenic factors;
    2. To evaluate the direct and indirect health outcomes associated with identified environmental risks;
    3. To raise awareness and educate the public about environmental health risks and preventive measures;
    4. To assess the effectiveness of proposed risk mitigation strategies in real-world settings; and
    5. To assess the vulnerability of different population groups, including socio-economic and demographic factors that may influence exposure and health outcomes.

    1.5 Research Questions

    The study came up with research questions so as to be able to ascertain the above stated objectives. The specific research questions for the study are stated below as follows:

    • Are public aware about environmental health risks and preventive measures?
    • What are the sources and distribution of environmental health risks, including natural and anthropogenic factors?
    • What is the direct and indirect health outcomes associated with identified environmental risks?
    • What is the effectiveness of proposed risk mitigation strategies in real-world settings?
    • What is the vulnerability of different population groups, including socio-economic and demographic factors that may influence exposure and health outcomes?

    1.6 Research Hypothesis

    In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.

    Hypothesis One

    • H0: Implementation of holistic and proactive risk mitigation strategies will not lead to more effective protection of public health against environmental hazards.
    • H1: Implementation of holistic and proactive risk mitigation strategies will lead to more effective protection of public health against environmental hazards.

    1.7 Significance of Study

    Upon the completion of this research, the findings of this study will inform evidence-based policy development and decision-making processes related to environmental health and risk mitigation. Policymakers can utilize the insights gained to design and implement regulations, interventions, and resource allocation strategies aimed at reducing environmental health risks and improving public health outcomes.

    Additionally, community involvement is essential for effective risk mitigation efforts, as communities often bear the brunt of environmental health risks and play a critical role in implementing solutions. This study underscores the significance of community engagement strategies, participatory approaches, and empowerment initiatives in promoting resilience and enhancing public health outcomes.

    Furthermore, assessing risk mitigation in public health for environmental health requires interdisciplinary collaboration among public health professionals, environmental scientists, policymakers, and community stakeholders. This study highlights the importance of interdisciplinary approaches and knowledge exchange in addressing complex environmental health challenges.

    Finally, this study will be of immense benefit to researchers who intend to know more on this study and can also be used by non-researchers to build more on their research work. This study contributes to knowledge and could serve as a guide for other study.


    1.8 Scope of the Study

    The scope of the research is focused on assessing the risk mitigation in public health for environmental health.


    1.9 Limitations of the Study

    During the course of this study, many things militated against its completion, some of which are:

    1. Time Constraint: The time frame given to accomplish this project was very short due to school academic calendar and it was carried out under pressure which made the researcher not to implement some necessary features.
    2. Research Material: availability of research material is a major setback to the scope of the study.
    3. Financial Constraint: Insufficient fund tends to impede the efficiency of the researcher in sourcing for the relevant materials, literature or information and in the process of data collection (internet, questionnaire and interview).

    1.10 Definition of Terms

    Risk Mitigation:

    Risk Mitigation refers to the process of identifying, assessing, and implementing measures to reduce the adverse effects of potential threats or hazards. in the context of public health, this involves strategies designed to minimize the impact of environmental health risks on populations (CDC, 2018).

    Public Health:

    Public Health is the science and art of preventing disease, prolonging life, and promoting health through organized community efforts. This includes a wide range of activities such as health education, disease prevention, health policy development, and the regulation of health services. Public health aims to improve the health outcomes of populations by addressing factors that can influence health, including environmental, social, and behavioral determinants (World Health Organization [WHO], 2021).

    Environmental Health:

    Environmental Health is a branch of public health focused on the study and management of environmental factors that can potentially affect human health. This includes both natural and human-made environments and encompasses a broad array of issues such as air and water quality, chemical exposures, climate change, and the built environment (National Institute of Environmental Health Sciences [NIEHS], 2019).

    Health Risk:

    Health Risk refers to the probability or likelihood that a person will experience a negative health outcome due to exposure to a hazardous agent or condition. Health risks can arise from various sources, including biological agents, chemicals, physical hazards, and environmental conditions (Environmental Protection Agency [EPA], 2017).

    Epidemiology:

    Epidemiology is the study of the distribution and determinants of health-related states or events in specified populations and the application of this study to control health problems. Epidemiologists seek to understand the patterns and causes of diseases and injuries in populations, and they use this information to develop and evaluate strategies for preventing and controlling health issues (Last, 2001).

    Exposure Assessment:

    Exposure Assessment is the process of measuring or estimating the intensity, frequency, and duration of human exposures to environmental agents. This involves identifying the sources of exposure, the pathways through which individuals may come into contact with hazards, and the populations that are affected. Accurate exposure assessment is critical for evaluating the potential health risks associated with environmental hazards (National Research Council, 1991).

    Intervention:

    Intervention in public health refers to any action or program that aims to improve health outcomes by preventing or reducing the risk of health problems. Interventions can be at various levels, including individual, community, and policy levels, and may involve educational, behavioral, medical, or environmental strategies (WHO, 2019).

    Vulnerability:

    Vulnerability in the context of environmental health refers to the degree to which a population or individual is susceptible to harm from exposure to environmental hazards. Factors contributing to vulnerability include age, gender, health status, socio-economic status, and geographic location. Identifying vulnerable populations is essential for targeting risk mitigation efforts effectively (Turner et al., 2003).
