1.1 Introduction
Poor sanitation refers to inadequate access to facilities and services for the safe disposal of human waste, improper management of solid waste, poor drainage systems, and limited access to safe water, all of which expose individuals and communities to preventable diseases. Diarrheal diseases remain one of the leading causes of illness and death, particularly among children under five years of age in developing countries. These diseases are commonly caused by bacterial, viral, and parasitic infections transmitted through contaminated food and water or poor personal hygiene. Diarrheal diseases are largely preventable through improved sanitation, access to safe drinking water, and proper hygiene practices. The availability of adequate sanitation facilities has therefore become a critical requirement for reducing the burden of communicable diseases and improving public health outcomes (World Health Organization, 2023).
As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the aim and objectives of the study. Others are significance of the study, scope of work, research hypothesis and questions, limitation of the study and definition of terms.
1.2 Background of Study
Historically, sanitation dates back to ancient civilizations where people recognized the importance of maintaining clean environments to prevent diseases. According to Rosen (2015), early civilizations such as those in Mesopotamia, Egypt, the Indus Valley, and Rome developed drainage systems, public baths, and sewage disposal facilities to improve environmental cleanliness and protect public health. The Roman Empire, in particular, constructed extensive sewer networks and aqueducts that supplied clean water and removed waste from urban settlements.
Sanitation according to the World Health Organization (2023) refers to the provision and maintenance of facilities and services that ensure the safe disposal of human waste, proper management of wastewater, and the promotion of hygienic practices that protect human health. Good sanitation is recognized as one of the most effective public health measures for preventing infectious diseases and improving the quality of life. Access to adequate sanitation facilities reduces human exposure to disease-causing microorganisms, prevents environmental contamination, and contributes to healthier communities. In many developing countries, however, sanitation services remain inadequate due to rapid urbanization, population growth, poor infrastructure, and weak environmental management systems.
Similarly, the United Nations Children's Fund (2023) reported that poor sanitation and inadequate hygiene remain major contributors to waterborne and foodborne diseases across low- and middle-income countries. Communities without proper toilet facilities often depend on unsafe methods of waste disposal, including open defecation and indiscriminate dumping of refuse. These practices contaminate water bodies, agricultural land, and residential environments, thereby increasing the spread of harmful microorganisms. Poor sanitation also affects access to safe drinking water because waste materials frequently seep into underground and surface water sources used for domestic purposes. As a result, residents of poorly serviced communities face a higher risk of diarrheal infections and other communicable diseases.
Furthermore, according to the United Nations Human Settlements Programme (2022), rapid urbanization has resulted in the expansion of slum settlements in many developing countries. These settlements are characterized by overcrowding, inadequate housing, poor drainage systems, limited access to potable water, and insufficient sanitation infrastructure. The combination of these conditions creates an environment where infectious diseases spread rapidly among residents. Slum communities often receive limited government attention in terms of sanitation infrastructure and waste management services, making them particularly vulnerable to outbreaks of diarrheal diseases and other environmentally related illnesses.
Mara, Lane, Scott, and Trouba (2018) asserted that contaminated water, food, and household environments serve as major routes through which disease-causing organisms spread within communities. Effective sanitation systems interrupt these transmission pathways and significantly reduce the burden of preventable diseases. Additionally, according to Prüss-Ustün et al. (2019), inadequate sanitation, unsafe drinking water, and poor hygiene account for a substantial proportion of the global burden of diarrheal diseases. The researchers affirmed that improved sanitation facilities, regular handwashing with soap, and access to safe water have consistently reduced disease transmission across different populations. They further stated that sanitation improvements not only protect individual health but also reduce healthcare costs, increase productivity, and improve educational outcomes by minimizing illness-related absenteeism.
Relatedly, according to the Federal Ministry of Environment (2021), environmental sanitation in Nigeria includes proper waste collection, effective drainage maintenance, safe disposal of human waste, and public participation in maintaining clean surroundings. The Ministry stated that environmental sanitation programmes are designed to reduce environmental pollution and improve public health. Despite these efforts, many communities continue to experience challenges associated with poor waste management, weak enforcement of sanitation regulations, and inadequate sanitation infrastructure (Federal Ministry of Environment, 2021).
This study is set against the backdrop of persistent sanitation challenges and the continued incidence of diarrheal diseases in selected slum communities in Kano State, Nigeria, with a view to assessing the impact of poor sanitation on the occurrence of diarrheal diseases and providing evidence that may support effective environmental sanitation and public health interventions.
1.3 Statement of Problems
Investigation revealed that poor sanitation remains a major public health challenge in many slum communities where inadequate toilet facilities, indiscriminate disposal of solid waste, blocked drainage systems, and limited access to safe drinking water continue to expose residents to disease-causing microorganisms (World Health Organization [WHO], 2023). These environmental conditions encourage the contamination of water sources, food, and living environments, thereby increasing the risk of diarrheal diseases among vulnerable populations.
In Kano State, the rapid growth of informal settlements has increased pressure on existing sanitation facilities and environmental health services. Many households still depend on unsafe water sources and inadequate waste disposal methods, while overcrowded living conditions further increase the transmission of diarrheal diseases (United Nations Human Settlements Programme [UN-Habitat], 2022). It is against this backdrop that this study seeks to assess the impact of poor sanitation on the incidence of diarrheal diseases in selected slum communities in Kano State, Nigeria.
1.4 Aim and Objectives of Study
The aim of this study is to assess the impact of poor sanitation on the incidence of diarrheal diseases in selected slum communities in Kano State, Nigeria. In achieving this aim, the following specific objectives were laid out as follows to:
- Examine the relationship between poor sanitation and the incidence of diarrheal diseases in slum communities.
- Determine the effect of improper solid waste disposal on the occurrence of diarrheal diseases.
- Assess the influence of access to safe water on the incidence of diarrheal diseases.
- Evaluate the effect of personal hygiene practices on the occurrence of diarrheal diseases among residents.
- Examine the role of environmental sanitation enforcement in reducing the incidence of diarrheal diseases.
1.5 Research Questions
The study came up with research questions so as to be able to ascertain the above stated objectives. The specific research questions for the study are stated below as follows:
- What relationship exists between poor sanitation and the incidence of diarrheal diseases in slum communities?
- What effect does improper solid waste disposal have on the occurrence of diarrheal diseases?
- How does access to safe water influence the incidence of diarrheal diseases?
- What effect do personal hygiene practices have on the occurrence of diarrheal diseases among residents?
- What role does environmental sanitation enforcement play in reducing the incidence of diarrheal diseases?
1.6 Research Hypotheses
In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.
Hypothesis One
- H0: There is no significant relationship between poor sanitation and the incidence of diarrheal diseases in slum communities.
- H1: There is a significant relationship between poor sanitation and the incidence of diarrheal diseases in slum communities.
Hypothesis Two
- H0: Improper solid waste disposal has no significant effect on the occurrence of diarrheal diseases.
- H1: Improper solid waste disposal has a significant effect on the occurrence of diarrheal diseases.
Hypothesis Three
- H0: Access to safe water has no significant influence on the incidence of diarrheal diseases.
- H1: Access to safe water has a significant influence on the incidence of diarrheal diseases.
Hypothesis Four
- H0: Personal hygiene practices have no significant effect on the occurrence of diarrheal diseases among residents.
- H1: Personal hygiene practices have a significant effect on the occurrence of diarrheal diseases among residents.
Hypothesis Five
- H0: Environmental sanitation enforcement has no significant role in reducing the incidence of diarrheal diseases.
- H1: Environmental sanitation enforcement has a significant role in reducing the incidence of diarrheal diseases.
1.7 Significance of Study
It is believed that at the completion of the study, the findings will provide reliable information on how poor sanitation contributes to the spread of diarrheal diseases in slum communities. The study will also support public health programmes aimed at improving sanitation and hygiene practices in affected communities.
Furthermore, the result obtained from this research will assist government agencies in planning effective environmental sanitation interventions. It will also provide useful information for future researchers interested in sanitation and disease prevention.
Lastly, the study will contribute to existing knowledge on environmental sanitation and diarrheal disease control in Nigeria.
1.8 Scope of Study
This study focuses on the impact of poor sanitation on the incidence of diarrheal diseases among residents of selected slum communities in Kano State, Nigeria. The study covers sanitation practices, waste disposal methods, access to safe drinking water, personal hygiene practices, environmental sanitation enforcement, and the occurrence of diarrheal diseases. The respondents comprise households living within the selected slum communities in Kano State.
1.9 Limitations of the Study
During the course of this study, there were some problems encountered which stood as limitations to the research work. Some of the limitations include:
- Some respondents were unwilling to provide complete information, which affected data collection.
- The period available for conducting the study was short, while financial resources available for transportation, printing, and other research activities were limited.
- Lastly, access to some areas was difficult because of poor road conditions, and relevant records on diarrheal disease cases were not always available from community sources.
1.10 Definition of Terms
Poor Sanitation:
Poor sanitation refers to the absence or inadequate provision of facilities and services for the safe disposal of human waste, solid waste, and wastewater, leading to unhealthy environmental conditions (World Health Organization [WHO], 2023).
Diarrheal Diseases:
Diarrheal diseases are illnesses characterized by the passage of three or more loose or watery stools within twenty-four hours, usually caused by infections transmitted through contaminated food, water, or poor hygiene (WHO, 2023).
Slum Community:
A slum community is a densely populated urban settlement where residents experience inadequate housing, poor sanitation, limited access to safe water, overcrowding, and insufficient basic social services (United Nations Human Settlements Programme [UN-Habitat], 2022).
Environmental Sanitation:
Environmental sanitation refers to activities and measures aimed at maintaining a clean environment through proper waste management, drainage maintenance, and pollution control to protect public health (Federal Ministry of Environment, 2021).
