Outcomes of Health Information Management in Reducing Infant Mortality


Health Information Management refers to the systematic process of collecting, storing, processing, and using health data to support effective healthcare delivery and improve patient outcomes, particularly in maternal and infant health services. The purpose of this study is to examine the outcomes of Health Information Management in reducing infant mortality at Warri Central Hospital by assessing data collection, record-keeping, digitization, staff competency, and operational challenges affecting health information use. The motivation for this research is based on persistent infant mortality cases linked to weak health data systems, incomplete records, delayed reporting, and limited digital integration in hospitals. Improving health information use is essential for strengthening infant care delivery and supporting timely clinical decisions.

Data were collected using structured questionnaires administered to 150 healthcare workers, including doctors, nurses, midwives, and health information officers at Warri Central Hospital. Secondary data from hospital reports were also used to support the study. The findings show that 40.0% of respondents rated data collection as effective and 23.3% very effective, while 36.7% reported moderate to low effectiveness. Record-keeping had 36.7% high impact and 26.7% very high impact on infant mortality. The system is 36.7% partially digitized and only 13.3% fully digitized. Furthermore, 33.3% reported very severe challenges and 30.0% severe challenges affecting HIM use, indicating ongoing system gaps that furthermore limit efficiency.

The outcome of this research indicates that Health Information Management at Warri Central Hospital is functional but not fully optimized for infant mortality reduction. Based on the result obtained from this research, it was recommended that the Delta State Ministry of Health should support the full digitization of the health information system at Warri Central Hospital by providing adequate funding, infrastructure, and technical support to reduce reliance on manual record systems and improve data integration. Also, the hospital management should strengthen the health information data collection system by ensuring that all patient information is accurately captured, properly documented, and promptly updated across all departments to improve data reliability for clinical decision-making.

1.1 Introduction

Health Information Management (HIM) is defined as the practice of collecting, analyzing, storing, and protecting patient health data in a way that ensures its accuracy, accessibility, and usefulness for healthcare decision-making (World Health Organization, 2023). It serves as a critical component of modern healthcare systems because it supports effective planning, monitoring, and evaluation of health services. In the context of maternal and child health, HIM plays an essential role in tracking health indicators that influence infant survival and overall population health outcomes.

Infant mortality refers to the death of children before their first birthday and is widely used as a key indicator of a country's health status and healthcare system performance. High infant mortality rates are often linked to inadequate access to quality healthcare services, poor maternal health, and weak health information systems that fail to support timely clinical decisions (UNICEF, 2022). In many developing countries, including Nigeria, infant mortality remains a persistent public health challenge despite ongoing interventions aimed at improving healthcare delivery.

As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the Aim and Objectives of the study. Others are significance of the study, scope of work, research hypothesis and questions, limitation of the study and definition of terms.


1.2 Background of Study

Health Information Management (HIM) is a core component of modern healthcare systems that focuses on the systematic collection, storage, retrieval, and use of health data to support clinical, administrative, and policy decisions. According to World Health Organization (2023), effective health information systems improve the quality of healthcare delivery by ensuring that accurate and timely data is available for decision making at all levels of the health system. Health Information Management is especially important in maternal and child health services, where timely and accurate information can determine survival outcomes for infants and mothers.

Infant mortality, defined as the death of a child before reaching one year of age, remains a major indicator of public health performance globally. According to UNICEF (2022), infant mortality rates are significantly higher in low- and middle-income countries due to factors such as poor access to quality healthcare, inadequate maternal care, and weak health information systems. In many developing countries, including Nigeria, infant mortality continues to pose a serious challenge despite various government and international interventions aimed at improving child health outcomes.

According to Federal Ministry of Health Nigeria (2021), Nigeria's healthcare system has continued to face challenges such as incomplete health records, poor data management systems, and inadequate use of health information for planning and decision making. The report further asserted that these gaps in health information management reduce the efficiency of healthcare delivery, particularly in critical areas such as neonatal and infant care. Without reliable health data, healthcare providers are limited in their ability to identify trends, monitor high-risk pregnancies, and respond effectively to complications that may lead to infant deaths.

World Health Organization (2023) stated that health information systems that are well structured and properly implemented contribute significantly to reducing preventable deaths by enabling early detection of health risks, improving referral systems, and strengthening continuity of care. The organization affirmed that countries with strong health information management frameworks tend to record lower infant mortality rates due to improved coordination of maternal and child health services.

UNICEF (2022) contends that the persistence of infant mortality in many regions is linked not only to medical factors but also to systemic issues such as poor data quality and lack of integration of health information systems. It further reported that when health data is fragmented or poorly managed, healthcare providers are unable to make informed decisions that could improve infant survival outcomes.

According to Adeniran and Okoye (2020), effective health information management in Nigerian hospitals is often hindered by inadequate infrastructure, shortage of trained health information personnel, and limited adoption of digital health technologies. They stated that these challenges contribute to delays in record keeping, data inconsistency, and poor utilization of health information for clinical decision making. As a result, many healthcare facilities struggle to implement evidence-based interventions aimed at reducing infant mortality.

Similarly, Oladipo (2019) asserted that the quality of health information directly influences the effectiveness of healthcare delivery systems. He emphasized that accurate and timely health data enables healthcare workers to monitor patient outcomes, identify health risks early, and implement appropriate interventions. In the context of infant health, such capabilities are essential for reducing preventable deaths caused by conditions such as birth complications, infections, and neonatal disorders.

This study is set against the backdrop of the need to evaluate how health information management practices influence infant mortality outcomes in Warri Central Hospital and contribute to broader improvements in healthcare delivery.


1.3 Statement of Problems

Investigation revealed that infant mortality remains a major public health concern in Nigeria, with preventable deaths of newborns and infants still occurring at alarming rates despite improvements in healthcare delivery systems. Health Information Management (HIM) is expected to play a critical role in reducing these deaths by ensuring accurate data collection, timely reporting, and effective use of health information for decision making in hospitals. However, in many health facilities, including secondary healthcare institutions such as Warri Central Hospital, gaps still exist in how health information is managed and utilized to improve infant health outcomes (World Health Organization, 2023).

Additionally, poor documentation practices, incomplete medical records, and delays in data reporting often affect the quality of healthcare decisions made by professionals (UNICEF, 2022). On the other hand, when health information is properly managed, it is expected to support early diagnosis, improve continuity of care, and enhance monitoring of infant health indicators, thereby reducing mortality rates.

Furthermore, the fragmented health data systems in many Nigerian hospitals result in poor coordination between departments, limiting the ability to track maternal and infant health trends effectively (Federal Ministry of Health Nigeria, 2021). It is against this backdrop that this study seeks to examine the outcomes of health information management in reducing infant mortality in Warri Central Hospital.


1.4 Aim and Objectives of Study

The aim of this study is to evaluate the outcomes of Health Information Management in reducing infant mortality at Warri Central Hospital, Delta State, Nigeria.

The specific objectives of this study include:

  1. To examine the effectiveness of health information data collection processes in Warri Central Hospital.
  2. To assess the impact of record-keeping practices on infant mortality management.
  3. To determine the level of digitalization of health information systems in the hospital.
  4. To evaluate the influence of staff competency on health information management outcomes.
  5. To identify challenges affecting the use of health information in reducing infant mortality.

1.5 Research Questions

Based on the stated objectives, the study will address the following research questions:

  • How effective are the health information data collection processes in Warri Central Hospital?
  • What is the impact of record-keeping practices on infant mortality outcomes?
  • To what extent is the health information system in the hospital digitized?
  • How does staff competency influence health information management outcomes?
  • What challenges affect the use of health information in reducing infant mortality?

1.6 Research Hypotheses

In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.

Hypothesis 1

  • H0: There is no significant relationship between health information data collection processes and reduction of infant mortality in Warri Central Hospital.
  • H1: There is a significant relationship between health information data collection processes and reduction of infant mortality in Warri Central Hospital.

Hypothesis 2

  • H0: Record-keeping practices have no significant impact on infant mortality outcomes in Warri Central Hospital.
  • H1: Record-keeping practices have a significant impact on infant mortality outcomes in Warri Central Hospital.

Hypothesis 3

  • H0: Digitalization of health information systems has no significant effect on infant mortality reduction.
  • H1: Digitalization of health information systems has a significant effect on infant mortality reduction.

Hypothesis 4

  • H0: Staff competency does not significantly influence health information management outcomes.
  • H1: Staff competency significantly influences health information management outcomes.

Hypothesis 5

  • H0: There is no significant relationship between staff competency and health information management outcomes in Warri Central Hospital.
  • H1: There is a significant relationship between staff competency and health information management outcomes in Warri Central Hospital.

1.7 Significance of Study

It is believed that at the completion of the study, the findings will provide Warri Central Hospital management with practical evidence on how health information management affects infant mortality outcomes and will guide improvements in data handling processes. Also, the findings will assist the Delta State Ministry of Health in strengthening health information policies aimed at improving maternal and child healthcare services.

Furthermore, the findings will support healthcare workers by improving understanding of how accurate health records will enhance infant care delivery. In addition, the research will help policymakers design better interventions for improving health data systems in public hospitals.

Lastly, the study will contribute to academic knowledge on health information systems and their role in reducing infant mortality in Nigeria.


1.8 Scope of Study

The study focuses on Warri Central Hospital in Delta State, Nigeria, and examines Health Information Management practices in relation to infant mortality reduction within the hospital's maternal and child health departments.


1.9 Limitations of the Study

During the course of this study, there were some problems encountered which stood as limitations to the research work. Some of the limitations include:

  1. Time Constraint: The time frame given to accomplish this project was very short due to school academic calendar and it was carried out under pressure which made the researcher not to implement some necessary features.
  2. Financial Constraint: Insufficient fund tends to impede the efficiency of the researcher in sourcing for the relevant materials, literature or information and in the process of data collection (internet, questionnaire and interview).
  3. Initial Cooperation Delay from Respondents: A particular limitation of this work came as a result of the respondent refusal to offer their cooperation at the initial time they were contacted. This contributed in making the success of this research study difficult.

Health Information Management:

Health Information Management refers to the practice of collecting, storing, analyzing, and using patient health data to support healthcare decision-making (World Health Organization, 2023). It ensures that accurate and timely information is available for improving healthcare delivery systems.

Infant Mortality:

Infant Mortality refers to the death of a child before reaching one year of age and is used as a key indicator of health system performance (UNICEF, 2022). It reflects the effectiveness of maternal and child healthcare services.

Health Information System:
