Poor Environmental Sanitation and Its Impact on Public Health Challenges in Communities


Environmental sanitation refers to the management of waste, drainage, and hygiene practices that ensure clean and safe living conditions in communities, reducing exposure to disease-causing agents and promoting public health. The purpose of this study is to examine the impact of poor environmental sanitation on public health challenges in communities by assessing causes, waste management systems, awareness levels, disease relationships, and improvement strategies. The motivation for this research arises from increasing environmental degradation and recurring disease outbreaks linked to poor sanitation in communities. Many areas experience inadequate waste disposal, weak enforcement of sanitation laws, and limited public awareness, leading to rising health risks. The outcome of this research will contribute to practical solutions that improve sanitation systems and reduce disease burden in affected communities.

Data were collected using structured questionnaires, observations, and secondary sources from environmental health records. A total of 200 respondents including household heads and health workers participated in the study, ensuring reliable and relevant information. The findings show that inadequate waste disposal facilities accounted for 25.0%, lack of awareness 20.0%, and poor law enforcement 17.5%. Furthermore, 30.0% rated waste systems as ineffective while 40.0% reported strong disease relationship with poor sanitation. Additionally, 27.5% identified waste facilities as key strategy and 22.5% supported awareness campaigns.

The results showed chi-square value of 18.76 and 21.34 significant at 0.05, correlation values of 0.68 and 0.74, and t-value of 4.21, indicating strong relationships between sanitation and health outcomes. The study concludes that poor environmental sanitation significantly increases public health challenges in communities, particularly through disease spread and ineffective waste management systems. Improved sanitation infrastructure, awareness, and enforcement are necessary for healthier communities. Based on the result obtained from this research, it was recommended that public health agencies should intensify awareness campaigns and health education programs to improve community knowledge of proper sanitation practices, hygiene behavior, and the health risks associated with poor environmental conditions.

1.1 Introduction

Poor environmental sanitation refers to the inadequate management and disposal of waste, lack of proper drainage systems, unsafe handling of human and animal excreta, and overall failure to maintain clean surroundings that are free from conditions capable of promoting disease transmission. It involves situations where solid waste accumulates in residential and public spaces, sewage is improperly disposed of, and clean water sources are contaminated due to human activities. According to the World Health Organization, environmental sanitation is a critical component of public health that focuses on maintaining hygienic conditions through proper waste disposal, safe water supply, and effective environmental management to prevent disease outbreaks (WHO, 2023). In many communities, poor environmental sanitation remains a major public health concern due to rapid urbanization, population growth, and inadequate infrastructure for waste management.

As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the Aim and Objectives of the study. Others are significance of the study, scope of work, research hypothesis and questions, limitation of the study and definition of terms.


1.2 Background of Study

Poor environmental sanitation has a long historical relationship with public health challenges in human settlements, dating back to early civilizations where the absence of organized waste disposal systems contributed to repeated outbreaks of infectious diseases. In ancient societies such as Mesopotamia, Egypt, and the Indus Valley, rudimentary drainage systems existed, but waste management remained largely uncoordinated, leading to contamination of water sources and widespread disease transmission. According to the World Health Organization, historical evidence shows that inadequate sanitation has consistently been linked to outbreaks of cholera, dysentery, and other waterborne diseases across different periods of human development (WHO, 2023). Poor environmental sanitation remains a major determinant of public health outcomes in both urban and rural communities worldwide. It refers to conditions where waste is improperly managed, drainage systems are inadequate, water sources are contaminated, and general environmental cleanliness is not maintained at a level that prevents disease transmission.

Prüss-Ustün et al. (2019) asserted that, inadequate sanitation, unsafe drinking water, and poor hygiene practices contribute significantly to the global burden of disease, particularly diarrhoeal diseases that remain among the leading causes of death in children under five years of age. The authors reported that millions of people are affected annually by illnesses linked to contaminated environments, especially in developing regions where sanitation infrastructure is weak or absent.

According to United Nations Children's Fund (UNICEF, 2022), poor sanitation practices are closely associated with preventable child mortality and morbidity, especially in communities where access to clean water and basic sanitation facilities is limited. UNICEF stated that children are disproportionately affected because they are more likely to be exposed to contaminated environments and have weaker immune systems. The organization affirmed that improving sanitation and hygiene practices is essential for reducing preventable deaths and improving overall child health outcomes globally.

According to Mara et al. (2010), sanitation is a fundamental public health intervention that directly influences disease transmission pathways. The authors contended that inadequate sanitation systems allow pathogens to spread through water, soil, and food, thereby increasing the incidence of infectious diseases such as cholera, typhoid fever, dysentery, and hepatitis A. They further explained that environmental contamination resulting from open defecation and improper waste disposal creates persistent public health risks in both rural and urban settlements.

Fewtrell and Colford (2005) articulated that, interventions in water, sanitation, and hygiene have a measurable impact on reducing diarrhoeal disease incidence. The authors asserted that communities lacking improved sanitation facilities experience significantly higher rates of infection compared to those with access to improved systems. In many developing communities, poor environmental sanitation is exacerbated by weak institutional frameworks and limited enforcement of environmental health regulations. According to Bartram and Cairncross (2010), hygiene and sanitation form the foundation of preventive health, yet they are often neglected in policy implementation and resource allocation. The authors affirmed that sustainable improvements in sanitation require coordinated efforts between governments, communities, and international development partners.

This study is set against the backdrop of increasing public health challenges associated with poor environmental sanitation in communities, where inadequate waste management, unsafe water sources, and poor hygiene practices continue to contribute significantly to disease burden and reduced quality of life.


1.3 Statement of Problems

Investigation revealed that poor sanitation conditions are linked to insufficient public awareness, weak environmental health enforcement, and limited access to waste disposal facilities. These conditions create an environment where disease causing organisms thrive, increasing the risk of outbreaks and sustained transmission of infections. The presence of open defecation sites, clogged waterways, and uncollected refuse often leads to contamination of water sources and food supplies.

On the other hand, communities that adopt improved sanitation practices tend to experience reduced incidence of sanitation related diseases, improved quality of life, and enhanced productivity among residents. However, such improvements are often hindered by poverty, rapid urbanization, and lack of coordinated environmental health policies. In several urban and rural communities, population growth outpaces sanitation infrastructure development, resulting in overcrowded living conditions and increased exposure to environmental health hazards.

Furthermore, poor environmental sanitation is also closely associated with the spread of vector borne diseases such as malaria and dengue fever due to stagnant water and unmanaged waste sites that serve as breeding grounds for mosquitoes. The United Nations Children's Fund emphasizes that unsafe sanitation and hygiene practices significantly increase the vulnerability of children and women, particularly in marginalized communities where access to clean water and sanitation services is limited (UNICEF, 2022). It is against this backdrop that this study seeks to examine poor environmental sanitation and its impact on public health challenges in communities.


1.4 Aim and Objectives of Study

The aim of this study is to assess the impact of poor environmental sanitation on public health challenges in communities.

The study is guided by the following objectives:

  1. To examine the causes of poor environmental sanitation in communities.
  2. To assess the existing waste management systems and their effectiveness.
  3. To determine the relationship between poor sanitation and disease prevalence.
  4. To evaluate the level of public awareness regarding environmental sanitation practices.
  5. To identify strategies for improving environmental sanitation and reducing health risks.

1.5 Research Questions

The study came up with research questions so as to be able to ascertain the above stated objectives. The specific research questions for the study are stated below as follows:

  • What are the causes of poor environmental sanitation in communities?
  • How effective are the existing waste management systems?
  • What is the relationship between poor environmental sanitation and disease prevalence?
  • What is the level of public awareness regarding environmental sanitation practices?
  • What strategies will improve environmental sanitation and reduce public health risks?

1.6 Research Hypothesis

In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.

Hypothesis One

  • H0: There is no significant relationship between poor environmental sanitation and disease prevalence in communities.
  • H1: There is a significant relationship between poor environmental sanitation and disease prevalence in communities.

Hypothesis Two

  • H0: Existing waste management systems do not significantly influence environmental sanitation conditions in communities.
  • H1: Existing waste management systems significantly influence environmental sanitation conditions in communities.

Hypothesis Three

  • H0: Public awareness does not significantly affect environmental sanitation practices in communities.
  • H1: Public awareness significantly affects environmental sanitation practices in communities.

1.7 Significance of Study

The outcome of this research will assist government health agencies will in improving sanitation policy implementation and enforcement. The study will also help local government authorities will in enhancing waste collection and drainage systems.

Furthermore, the study will support healthcare workers will in understanding environmental causes of disease outbreaks. In addition, the study will guide community members will in adopting better sanitation and hygiene practices.

Lastly, the study will benefit environmental health organizations will in designing targeted intervention programs.


1.8 Scope and Limitations of the Study

The study covers environmental sanitation practices, waste management systems, and their impact on public health in selected communities in Ogun State, Nigeria. It focuses on household sanitation behaviors, municipal waste disposal systems, and disease prevalence linked to environmental conditions.

The study was limited to selected communities in Ogun State and was not extended to other states in Nigeria due to geographical and logistical constraints.


1.9 Definition of Terms

Environmental Sanitation:

Environmental Sanitation refers to the maintenance of clean and hygienic conditions through proper waste disposal, drainage management, and control of environmental hazards that affect human health. According to World Health Organization (WHO, 2023), it involves interventions that reduce exposure to disease-causing agents in the environment.

Waste Management:

Waste Management refers to the collection, transportation, treatment, and disposal of waste materials. According to Tchobanoglous and Kreith (2002), effective waste management reduces environmental pollution and health risks.

Communicable Diseases:

Communicable Diseases refer to illnesses caused by infectious agents that can be transmitted from one person to another. According to Centers for Disease Control and Prevention (CDC, 2022), these diseases spread through contaminated water, food, air, or vectors.
