1.1 Introduction
Alcohol consumption during pregnancy refers to the intake of alcoholic beverages by a woman during the gestational period, exposing the developing fetus to alcohol through the placenta. According to the World Health Organization (2022), no amount of alcohol has been proven safe during pregnancy because alcohol can adversely affect fetal growth, organ formation, and neurological development. Pregnancy represents a critical stage in human development during which the fetus depends entirely on maternal health and behavior for optimal growth. Consequently, maternal alcohol consumption has become a major public health issue due to its association with preventable birth defects and developmental disorders.
Globally, alcohol use remains one of the most commonly consumed psychoactive substances. While many women discontinue alcohol use upon becoming pregnant, a significant proportion continue consumption either knowingly or unknowingly during the early stages of pregnancy (Popova et al., 2019). As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the aim and objectives of the study. Others are significance of the study, scope of work, research hypothesis and questions, limitation of the study and definition of terms.
1.2 Background of Study
Pregnancy is a critical period in human development during which maternal behaviors and lifestyle choices significantly influence fetal growth and overall pregnancy outcomes. Alcohol consumption during pregnancy has attracted considerable attention from healthcare professionals, researchers, and policymakers because of its harmful effects on both the mother and the developing fetus. According to the World Health Organization, alcohol is a teratogenic substance capable of crossing the placental barrier and reaching the fetus, where it can interfere with normal growth and organ development. The developing fetal brain is particularly vulnerable to alcohol exposure because fetal metabolic systems are not sufficiently mature to process alcohol effectively. As a result, prenatal alcohol exposure has become a major public health concern worldwide (World Health Organization, 2022).
May et al. (2018), prenatal alcohol exposure is among the leading preventable causes of developmental disabilities and birth defects. The researchers reported that alcohol consumption during pregnancy contributes to a range of adverse outcomes, including miscarriage, stillbirth, low birth weight, premature delivery, and congenital abnormalities. These outcomes affect not only the physical health of the child but also cognitive, emotional, and behavioral development. The increasing recognition of these risks has led to intensified efforts by health organizations to discourage alcohol use during pregnancy (May et al., 2018).
Similarly, Popova et al. (2019) stated that alcohol use during pregnancy remains prevalent in many regions of the world despite widespread public health campaigns. The authors reported that millions of pregnancies are exposed to alcohol annually, creating a substantial burden on healthcare systems and societies. Their findings indicated that women may continue alcohol consumption during pregnancy due to inadequate awareness, social acceptance of drinking, peer influence, and misconceptions regarding safe levels of alcohol intake. In the same vein, the Centers for Disease Control and Prevention (2023) affirmed that no known amount of alcohol consumption is considered safe during pregnancy. The agency reported that alcohol exposure can occur at any stage of pregnancy and may affect fetal development even before a woman becomes aware that she is pregnant. Consequently, health professionals advocate complete abstinence from alcohol throughout pregnancy to eliminate preventable risks associated with fetal exposure.
Likewise, Streissguth (2007) asserted that prenatal alcohol exposure has lifelong implications that extend beyond infancy and childhood. The scholar reported that children exposed to alcohol in the womb may experience difficulties in learning, memory, attention, communication, and social interaction. These impairments often persist into adolescence and adulthood, limiting educational attainment, employment opportunities, and overall quality of life. Such findings underscore the importance of preventing alcohol exposure during fetal development.
Furthermore, according to the National Institute on Alcohol Abuse and Alcoholism (2023), alcohol consumption during pregnancy is strongly associated with a group of conditions known as Fetal Alcohol Spectrum Disorders (FASD). The institute reported that these disorders encompass a range of physical, cognitive, and behavioral impairments resulting from prenatal alcohol exposure. Individuals affected by FASD may exhibit growth deficiencies, facial abnormalities, poor impulse control, learning disabilities, and mental health challenges. The long-term nature of these conditions places considerable demands on healthcare, educational, and social support systems.
O'Leary et al. (2010) contended that maternal alcohol consumption during pregnancy remains a complex public health issue because it is influenced by multiple social and environmental factors. The researchers reported that maternal age, educational level, socioeconomic status, cultural beliefs, and access to healthcare services can influence drinking behavior during pregnancy (O'Leary et al., 2010). Correspondingly, Lange et al. (2017) stated that the burden of alcohol-related developmental disorders is not evenly distributed across populations. The researchers reported that communities with limited healthcare access and lower levels of maternal health education tend to experience higher rates of prenatal alcohol exposure.
According to the United Nations Children's Fund (UNICEF, 2022), maternal health remains a priority area in many developing countries because preventable risk factors continue to contribute to poor pregnancy outcomes. The organization reported that public health interventions focusing on maternal education and behavior modification have been effective in improving neonatal and child health indicators. Such interventions are particularly important in addressing alcohol use during pregnancy because behavioral changes can directly reduce fetal exposure to harmful substances.
Consistent with this view, Adeoye and Bello (2021) affirmed that alcohol consumption among pregnant women remains a concern in some urban areas of Nigeria. The researchers reported that social gatherings, cultural practices, and inadequate awareness regarding fetal risks contribute to alcohol use during pregnancy. Their findings suggest that many women underestimate the potential consequences of alcohol exposure on fetal development, thereby increasing the likelihood of adverse birth outcomes. Relatedly, Eze and Okafor (2019) reported that antenatal care services provide a valuable platform for educating pregnant women about factors that may affect fetal development. The researchers asserted that routine counseling during antenatal visits can increase awareness of alcohol-related risks and encourage behavioral changes that support healthy pregnancies.
This study is set against the backdrop of increasing concerns regarding alcohol consumption during pregnancy and its effects on fetal development and long-term health outcomes among children.
1.3 Statement of Problems
Investigation revealed that alcohol consumption during pregnancy remains a significant public health concern across many countries, including Nigeria. Alcohol is capable of crossing the placental barrier and reaching the developing fetus, thereby exposing the unborn child to substances that may interfere with normal growth and development. The World Health Organization (2022) reported that prenatal exposure to alcohol is associated with adverse pregnancy outcomes, including miscarriage, stillbirth, low birth weight, premature birth, and developmental abnormalities. Although awareness campaigns have increased over the years, a considerable number of women continue to consume alcohol during pregnancy due to inadequate knowledge, social influences, cultural practices, and misconceptions regarding its safety.
Furthermore, prenatal alcohol exposure contributes to a wide range of physical, cognitive, behavioral, and neurological disorders collectively referred to as Fetal Alcohol Spectrum Disorders (FASD) (Centers for Disease Control and Prevention [CDC], 2023). Children affected by prenatal alcohol exposure often experience learning difficulties, poor academic performance, attention deficits, impaired memory, and social adjustment problems that may persist throughout life (May et al., 2018). It is against this backdrop that this study seeks to investigate the effects of alcohol consumption during pregnancy on fetal development and long-term child health outcomes.
1.4 Purpose of the Study
The purpose of this study is to examine the risks of alcohol consumption during pregnancy and its impacts on fetal development and long-term health outcomes in Lagos State, Nigeria, with a view to improving maternal health awareness, strengthening antenatal interventions, and reducing cases of alcohol-related birth complications.
1.5 Aim and Objectives of Study
The aim of this study is to investigate the effects of alcohol consumption during pregnancy on fetal development and long-term child health outcomes. In achieving this aim, the following specific objectives were laid out as follows:
- To assess the level of awareness of pregnant women on the risks of alcohol consumption during pregnancy.
- To determine the prevalence of alcohol use among pregnant women attending antenatal clinics in Lagos State.
- To examine the effects of alcohol consumption on fetal development outcomes.
- To identify long-term health consequences of prenatal alcohol exposure on children.
- To evaluate the effectiveness of antenatal education in reducing alcohol consumption during pregnancy.
