1.1 Introduction
Pain is defined as an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage, making it a subjective phenomenon that varies widely among individuals (International Association for the Study of Pain, 2020). In clinical practice, especially within emergency care settings, pain is regarded as the “fifth vital sign” due to its critical role in guiding diagnosis, treatment decisions, and overall patient management. Effective pain assessment therefore forms the foundation of quality nursing care, as it ensures that patients' discomfort is accurately identified and appropriately managed.
In emergency departments, nursing staff are often the first point of contact for patients presenting with acute conditions, trauma, or life-threatening injuries. This places them in a key position to assess and document pain using standardized tools such as the Numerical Rating Scale, Visual Analogue Scale, and Faces Pain Scale. These tools are designed to improve objectivity in pain measurement and enhance consistency in clinical decision-making (Herr et al., 2011).
As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the Aim and Objectives of the study. Others are significance of the study, scope of work, research hypothesis and questions, limitation of the study and definition of terms.
1.2 Background of Study
Pain is widely recognized as one of the most common and distressing symptoms experienced by patients seeking healthcare services, particularly in emergency departments where conditions are often acute, unpredictable, and life-threatening. According to the International Association for the Study of Pain (IASP, 2020), pain is defined as an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage. Herr et al. (2011) reported that despite the availability of validated pain assessment tools, their consistent use in clinical practice remains limited, particularly in acute care settings.
According to the World Health Organization (2020), effective pain management is a fundamental human right and an essential component of quality healthcare delivery. The organization affirmed that untreated or poorly managed pain can lead to prolonged hospital stays, increased healthcare costs, psychological distress, and reduced patient satisfaction. In emergency nursing practice, failure to properly assess pain can result in under-treatment or over-treatment, both of which have significant clinical implications.
American Nurses Association (2018) stated that nurses play a central role in pain assessment and management, as they are often the first healthcare providers to interact with patients in emergency situations. The association further stated that standardized pain assessment tools should be integrated into routine nursing practice to ensure accurate documentation and effective communication among healthcare professionals. However, it contends that barriers such as workload pressure, limited staffing, and insufficient training often hinder effective implementation.
Ferrell, McGuire, and Donovan (2015) affirmed that pain assessment remains one of the most challenging aspects of nursing practice, particularly in emergency care environments where patient conditions change rapidly. They affirmed that the use of standardized tools improves clinical outcomes, yet many nurses still rely on subjective judgment due to time constraints and institutional limitations. Furthermore, studies by Gregory and Haigh (2008) contended that although nurses possess adequate knowledge of pain assessment tools, there is often a gap between knowledge and practice. They contended that institutional culture, lack of continuous professional development, and absence of enforcement policies contribute to poor adherence to standardized pain assessment protocols.
In the context of the Federal Medical Centre, Yenagoa, Bayelsa State, emergency nursing staff operate under similar constraints, which may influence their ability to consistently apply standardized pain assessment tools. According to Ede and Uzochukwu (2021), tertiary healthcare institutions in Nigeria often face systemic challenges such as inadequate training programs, insufficient clinical guidelines reinforcement, and lack of audit systems for monitoring nursing practices. On the other hand, advancements in nursing education and global emphasis on evidence-based practice have led to increased awareness of standardized pain assessment tools. This study is set against the backdrop of increasing global advocacy for improved pain management practices, the persistent inconsistency in the use of standardized pain assessment tools among emergency nursing staff, and the need to enhance patient-centered care in tertiary healthcare institutions in Nigeria.
1.3 Statement of Problems
Investigation revealed that emergency nursing staff operate under demanding conditions characterized by high patient turnover, limited time, and critical case presentations. These conditions often influence how pain is evaluated, documented, and managed. Although several standardized tools such as the Numerical Rating Scale, Visual Analogue Scale, and Faces Pain Scale exist to improve accuracy, their consistent application is not always observed in practice.
Furthermore, inadequate pain assessment is not only a clinical issue but also a patient satisfaction and ethical concern, as unmanaged pain is associated with prolonged recovery time, increased psychological distress, and reduced trust in healthcare services (World Health Organization, 2020). In emergency care settings, where rapid and accurate assessment is critical, the inconsistency in pain measurement practices presents a significant gap in nursing care delivery. It is against this backdrop that this study seeks to examine the assessment of pain assessment tools used by emergency nursing staff at the Federal Medical Centre, Yenagoa, Bayelsa State.
1.4 Aim and Objectives of Study
The aim of this study is to assess the pain assessment tools used by emergency nursing staff at the Federal Medical Centre, Yenagoa. In achieving this aim, the following specific objectives were laid out as follows:
- To examine the types of pain assessment tools used by emergency nursing staff in the Federal Medical Centre, Yenagoa.
- To determine the level of awareness of standardized pain assessment tools among emergency nursing staff.
- To assess the frequency of use of pain assessment tools in emergency nursing practice.
- To identify factors affecting the utilization of pain assessment tools in the emergency unit.
- To evaluate challenges associated with effective pain assessment in emergency nursing care.
1.5 Research Questions
The study came up with research questions so as to be able to ascertain the above stated objectives. The specific research questions for the study are stated below as follows:
- What types of pain assessment tools are used by emergency nursing staff at the Federal Medical Centre, Yenagoa?
- What is the level of awareness of standardized pain assessment tools among emergency nursing staff?
- How frequently are pain assessment tools used in emergency nursing practice?
- What factors affect the utilization of pain assessment tools in the emergency unit?
- What challenges are associated with effective pain assessment in emergency nursing care?
1.6 Research Hypothesis
In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.
- H0: There is no significant relationship between the use of standardized pain assessment tools and effective pain management among emergency nursing staff at the Federal Medical Centre, Yenagoa.
- H1: There is a significant relationship between the use of standardized pain assessment tools and effective pain management among emergency nursing staff at the Federal Medical Centre, Yenagoa.
1.7 Significance of Study
The outcome of this study will assist nursing management at Federal Medical Centre, Yenagoa in improving supervision and standardization of pain assessment practices among emergency staff. It will also help emergency nursing staff enhance their understanding and application of standardized pain assessment tools in clinical practice
Furthermore, the research will support hospital administrators in developing policies that will improve patient pain documentation and care quality In addition, the findings of this research will provide policymakers with evidence that will guide improvement of nursing standards in emergency healthcare delivery
Lastly, this study will contribute to academic researchers by providing empirical data that will support future studies on pain management practices
1.8 Scope of Study
The scope of the research is focused on the assessment of pain assessment tools used by emergency nursing staff, using Federal Medical Centre, Yenagoa, Bayelsa State as a case study. The research scope will be restricted to only the emergency department and will not cover other units within the hospital.
1.9 Limitations of the Study
During the course of this study, there were some problems encountered which stood as limitations to the research work. Some of the limitations include:
- Time Constraint: The time frame given to accomplish this project was very short due to school academic calendar and it was carried out under pressure which made the researcher not to implement some necessary features.
- Financial Constraint: Insufficient fund tends to impede the efficiency of the researcher in sourcing for the relevant materials, literature or information and in the process of data collection (internet, questionnaire and interview).
- Initial Cooperation Delay from Respondents: A particular limitation of this work came as a result of the respondent refusal to offer their cooperation at the initial time they were contacted. This contributed in making the success of this research study difficult.
1.10 Definition of Terms
Pain Assessment:
Pain assessment refers to the systematic process of evaluating a patient's pain intensity, location, and characteristics using standardized tools or clinical judgment. According to McCaffery and Pasero (1999), pain is best assessed through patient self-report, making structured assessment essential for accurate clinical interpretation.
Pain Assessment Tools:
Pain assessment tools are standardized instruments used to measure pain intensity and characteristics in patients. According to Herr et al. (2011), commonly used tools include the Numerical Rating Scale, Visual Analogue Scale, and Faces Pain Scale, which improve objectivity in clinical pain evaluation.
Emergency Nursing Staff:
Emergency nursing staff are registered nurses who provide immediate care to patients in emergency departments. According to the American Nurses Association (2018), they play a critical role in rapid assessment, stabilization, and management of patients in acute conditions.
Standardized Pain Assessment:
Standardized pain assessment refers to the use of uniform tools and procedures to evaluate pain consistently across patients. The World Health Organization (2020) affirmed that standardized assessment improves accuracy, communication, and patient outcomes in healthcare delivery.
Emergency Department:
The emergency department is a specialized hospital unit that provides urgent treatment to patients with life-threatening or acute medical conditions. According to the International Association for the Study of Pain (2020), effective pain assessment in such settings is essential due to the severity and urgency of patient conditions.
