1.1 Introduction
Infection prevention and control refers to the set of evidence-based practices and procedures designed to prevent the spread of infectious diseases within healthcare environments, thereby protecting patients, healthcare workers, and visitors from avoidable infections (World Health Organization, 2022). Hand hygiene, which includes handwashing with soap and water or the use of alcohol-based hand rubs, is considered the most effective and simplest method of reducing the transmission of pathogens in clinical settings and is central to infection prevention strategies worldwide (Centers for Disease Control and Prevention, 2021).
Knowledge, attitudes, and practices (KAP) in the context of infection prevention refer to what healthcare workers know about infection control measures, how they feel about their importance, and how consistently they apply them in clinical practice. Adequate knowledge of infection prevention guidelines is expected to positively influence attitudes and improve compliance with recommended practices; however, evidence shows that this relationship is often inconsistent in real healthcare environments (Allegranzi & Pittet, 2009).
As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the Aim and Objectives of the study. Others are significance of the study, scope of work, research hypothesis and questions, limitation of the study and definition of terms.
1.2 Background of Study
Infection prevention and control is a fundamental aspect of healthcare delivery that focuses on the prevention of healthcare-associated infections through standardized procedures, protocols, and behavioral practices among healthcare workers. According to the World Health Organization (WHO, 2022), infection prevention and control refers to evidence-based practices designed to reduce the risk of transmission of infectious agents in healthcare settings, thereby ensuring the safety of patients, healthcare workers, and visitors. One of the most critical components of infection prevention is hand hygiene, which involves the use of soap and water or alcohol-based hand rubs to eliminate transient microorganisms that may be transmitted during patient care activities (World Health Organization, 2022).
According to the Centers for Disease Control and Prevention (2021), hand hygiene is recognized as the single most important measure for preventing the spread of infections in healthcare environments. The CDC further reported that proper hand hygiene practices significantly reduce the incidence of healthcare-associated infections and improve overall patient outcomes (CDC, 2021).
Allegranzi and Pittet (2009) stated that, healthcare-associated infections represent a major public health challenge that affects millions of patients annually, particularly in low- and middle-income countries. They asserted that inadequate hand hygiene practices among healthcare workers are a primary contributor to the transmission of infectious agents within clinical settings. They further contended that improving compliance with hand hygiene guidelines could reduce infection rates by a substantial margin, especially in resource-limited healthcare environments.
According to Sax et al. (2007), adherence to hand hygiene protocols is influenced by multiple factors, including knowledge level, institutional culture, availability of resources, and individual attitudes toward infection prevention. They reported that even when healthcare workers are aware of infection prevention guidelines, actual practice may still be poor due to behavioral and systemic barriers (Sax et al., 2007).
Whitby et al. (2006) affirmed that, healthcare workers often underestimate the importance of hand hygiene during routine clinical activities, particularly when there is low perceived risk of infection transmission. They stated that this perception gap contributes to poor adherence to recommended infection prevention practices. Similarly, they affirmed that improving awareness and reinforcing behavioral change strategies are essential in enhancing compliance levels among healthcare workers.
According to Allegranzi et al. (2010), the implementation of the WHO multimodal hand hygiene improvement strategy has significantly improved compliance rates in several healthcare facilities globally. They reported that interventions such as training, reminders in the workplace, availability of alcohol-based hand rubs, and institutional safety climate improvements are effective in promoting better infection prevention practices. However, they contended that sustainability of these improvements requires continuous monitoring and reinforcement. This study is set against the backdrop of the persistent global and local challenges of healthcare-associated infections and the need to improve hand hygiene compliance among healthcare workers in order to enhance patient safety and healthcare quality in Warri Central Hospital.
1.3 Statement of Problems
Investigation revealed that infection prevention and control remains a critical component of modern healthcare delivery due to the persistent burden of healthcare-associated infections, which continue to affect patient safety, increase hospitalization periods, and raise treatment costs globally. Hand hygiene is widely recognized as one of the most effective measures for preventing the transmission of infectious agents within healthcare settings, particularly among healthcare workers who are in constant contact with patients and contaminated surfaces (World Health Organization, 2022).
Furthermore, even where infection prevention policies and guidelines are available within healthcare institutions, actual adherence among healthcare workers is often low due to attitudinal and systemic barriers (WHO, 2022). In addition, inadequate reinforcement mechanisms, limited supervision, and inconsistent availability of alcohol-based hand rubs and clean water further worsen compliance levels among healthcare workers. It is against this backdrop that this study seeks to examine the knowledge, attitudes, and practices of healthcare workers on infection prevention and hand hygiene in Warri Central Hospital.
1.4 Aim and Objectives of Study
The aim of this study is to assess the knowledge, attitudes, and practices of healthcare workers on infection prevention and hand hygiene in Warri Central Hospital.
The specific objectives of this study include:
- To assess the level of knowledge of healthcare workers on infection prevention and hand hygiene in Warri Central Hospital.
- To examine the attitudes of healthcare workers toward infection prevention and hand hygiene practices.
- To evaluate the infection prevention and hand hygiene practices among healthcare workers in Warri Central Hospital.
- To identify factors influencing compliance with infection prevention and hand hygiene guidelines among healthcare workers.
- To determine the relationship between knowledge, attitudes, and practices of healthcare workers in Warri Central Hospital.
1.5 Research Questions
Based on the stated objectives, the study will address the following research questions:
- What is the level of knowledge of healthcare workers on infection prevention and hand hygiene in Warri Central Hospital?
- What are the attitudes of healthcare workers toward infection prevention and hand hygiene practices?
- What are the infection prevention and hand hygiene practices among healthcare workers in Warri Central Hospital?
- What factors influence compliance with infection prevention and hand hygiene guidelines among healthcare workers?
- What is the relationship between knowledge, attitudes, and practices of healthcare workers in Warri Central Hospital?
1.6 Research Hypotheses
In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.
Hypothesis 1
- H0: There is no significant relationship between knowledge and infection prevention practices among healthcare workers in Warri Central Hospital.
- H1: There is a significant relationship between knowledge and infection prevention practices among healthcare workers in Warri Central Hospital.
Hypothesis 2
- H0: There is no significant relationship between attitudes and infection prevention practices among healthcare workers in Warri Central Hospital.
- H1: There is a significant relationship between attitudes and infection prevention practices among healthcare workers in Warri Central Hospital.
Hypothesis 3
- H0: There is no significant relationship between knowledge, attitudes, and practices of healthcare workers in Warri Central Hospital.
- H1: There is a significant relationship between knowledge, attitudes, and practices of healthcare workers in Warri Central Hospital.
1.7 Significance of Study
The outcome of this research will provide evidence-based information that will improve infection prevention strategies in Warri Central Hospital. The study will also contribute to improving patient safety by providing evidence that will guide infection control interventions and strengthen existing hospital hygiene policies.
Furthermore, the research will assist in reducing the rate of healthcare-associated infections through improved practices. In addition, this study will support healthcare administrators in strengthening training programs on hand hygiene compliance.
Lastly, the outcome of this research will serve as a reference material for future researchers studying infection prevention practices in healthcare facilities within Nigeria and similar resource-limited settings.
1.8 Scope and Limitations of the Study
The scope of the research is focused on the knowledge, attitudes, and practices on infection prevention and hand hygiene among health care workers.
The study covers infection prevention and hand hygiene practices among healthcare workers in Warri Central Hospital, Delta State, Nigeria, concentrating on behavioral and institutional factors influencing compliance.
1.9 Definition of Terms
Infection Prevention and Control:
Infection prevention and control refers to evidence-based practices aimed at reducing the transmission of infections in healthcare settings through standardized procedures such as sterilization, hygiene, and use of protective measures (World Health Organization, 2022).
Hand Hygiene:
Hand hygiene refers to the act of cleaning hands using soap and water or alcohol-based hand rubs to remove microorganisms and prevent infection transmission (Centers for Disease Control and Prevention, 2021).
Knowledge:
Knowledge refers to the awareness and understanding healthcare workers have regarding infection prevention guidelines and procedures.
Attitudes:
Attitudes refer to the beliefs and perceptions of healthcare workers toward infection prevention and hand hygiene practices.
Practices:
Practices refer to the actual implementation of infection prevention and hand hygiene procedures by healthcare workers during patient care.
Healthcare Workers:
Healthcare workers refer to individuals such as doctors, nurses, and laboratory staff involved in patient care and hospital services.
