1.0 Introduction
1.1 Background of Study
Hypertension, commonly known as high blood pressure, is a major public health issue worldwide, contributing significantly to the burden of cardiovascular diseases (CVDs). The World Health Organization (WHO) reports that hypertension is responsible for an estimated 7.5 million deaths annually, about 12.8% of all deaths worldwide. Effective management of hypertension is critical to reducing the risk of heart attacks, strokes, and other related health complications (World Health Organization, 2019).
The relationship between lifestyle factors and hypertension has long been recognized in medical research. in the mid-20th century, studies began to highlight the impact of diet, physical activity, and other lifestyle choices on blood pressure. The landmark Framingham Heart Study, initiated in 1948, was one of the first to systematically investigate the epidemiology of hypertension and its association with lifestyle factors, setting the stage for subsequent research in this area.
By the late 20th century, there was a growing consensus in the medical community about the importance of lifestyle modifications in managing hypertension. In 1997, the National Institutes of Health (NIH) funded the Dietary Approaches to Stop Hypertension (DASH) study, which demonstrated that dietary changes, particularly a diet rich in fruits, vegetables, and low-fat dairy products, could significantly reduce blood pressure levels. This study was pivotal in establishing dietary modification as a cornerstone of hypertension management.
Appel et al. (2011) asserted that, Lifestyle modifications, such as dietary changes, increased physical activity, weight management, and smoking cessation, are essential components of hypertension management. Studies have shown that these modifications can significantly lower blood pressure and improve overall cardiovascular health. However, many hypertensive patients lack the knowledge and skills needed to implement these changes effectively. A structured training programme can play a crucial role in educating hypertensive patients about lifestyle modifications. Previous research has demonstrated that educational interventions can lead to significant improvements in patients' adherence to lifestyle modifications and, consequently, better blood pressure control (Appel et al. 2011).
Effective management of hypertension involves both pharmacological and non-pharmacological strategies. Lifestyle modifications, such as adopting a healthy diet, engaging in regular physical activity, reducing alcohol intake, quitting smoking, and maintaining a healthy weight, are fundamental non-pharmacological approaches recommended for controlling high blood pressure. Evidence suggests that these lifestyle changes can lead to substantial reductions in blood pressure levels and overall cardiovascular risk (Khatib, R., et al. 2014).
Despite the known benefits of lifestyle modifications, many hypertensive patients struggle to implement and maintain these changes. Lack of knowledge, motivation, and support are common barriers. Health education and structured training programmes have been identified as effective methods to enhance patients' understanding and adherence to recommended lifestyle modifications. Studies have shown that educational interventions can lead to significant improvements in patients' knowledge, attitudes, and practices regarding hypertension management (Miller et al. 2013).
Out-patient clinics serve as critical points of contact for hypertensive patients, providing opportunities for regular monitoring and support. Integrating training programmes into these settings can potentially enhance patients' ability to manage their condition effectively. This study focuses on assessing the impact of a structured training programme on the knowledge and practice of lifestyle modifications among hypertensive patients attending out-patient clinics. By doing so, the study aims to provide valuable insights into the effectiveness of educational interventions in improving health outcomes for individuals with hypertension (Puska et al. 2009).
Therefore, in Nigeria where the research was carried out, the activities that was conducted is to know the Effect of a training programme on knowledge and practice of lifestyle modification among hypertensive patients attending out-patient clinics.
1.2 Statement of Problems
Investigation revealed that hypertension remains one of the most significant public health challenges globally, contributing to a substantial burden of cardiovascular diseases, strokes, and renal complications. Despite advancements in medical treatments, achieving optimal blood pressure control continues to be problematic for many patients. This issue is compounded by the low adherence rates to prescribed lifestyle modifications, which are crucial for effective hypertension management (World Health Organization, 2019).
Many hypertensive patients lack sufficient knowledge about the importance of lifestyle changes, such as dietary modifications, regular physical activity, weight management, and the reduction of alcohol and tobacco use. This lack of knowledge often leads to poor implementation of these essential practices, resulting in suboptimal health outcomes. Additionally, the healthcare system frequently fails to provide adequate support and motivation for patients to adopt and maintain these lifestyle changes. Consequently, there is a critical need for effective intervention strategies that address these gaps in knowledge and practice (Chobanian et al., 2003).
Training programs designed to enhance patients' understanding and implementation of lifestyle modifications has shown promise in improving hypertension management. However, the effectiveness of these programs varies, and there is limited research specifically evaluating their impact on hypertensive patients attending out-patient clinics. Without robust evidence, it remains challenging to justify the integration of such programs into routine clinical practice fully (Krousel-Wood et al., 2004). It is against the backdrop that this study seeks to address these problems by evaluating the effect of a structured training program on the knowledge and practice of lifestyle modifications among hypertensive patients attending out-patient clinics.
1.3 Aim and Objectives of Study
The aim of the study is to determine the effect of a training programme on knowledge and practice of lifestyle modification among hypertensive patients attending out-patient clinics. In achieving this aim, the following specific objectives were laid out as follows:
- To assess the baseline knowledge of hypertensive patients regarding lifestyle modifications necessary for managing hypertension;
- To evaluate the change in knowledge of lifestyle modifications among hypertensive patients after participation in the training program;
- To determine the baseline practices related to lifestyle modifications among hypertensive patients;
- To identify barriers and facilitators to implementing lifestyle modifications among hypertensive patients; and
- To assess the change in the practice of lifestyle modifications among hypertensive patients following the training program.
1.4 Research Questions
The study came up with research questions so as to be able to ascertain the above stated objectives. The specific research questions for the study are stated below as follows:
- Is there a significant change in the blood pressure levels of hypertensive patients after participating in the training program?
- Do hypertensive patients face barriers in implementing lifestyle modifications?
- What is the baseline knowledge of hypertensive patients regarding lifestyle modifications for managing hypertension?
- What is the baseline practices related to lifestyle modifications among hypertensive patients?
- How effective is a structured training program in enhancing the practice of lifestyle modifications among hypertensive patients?
- What facilitators support the implementation of lifestyle modifications among hypertensive patients?
- How effective is a structured training program in improving the knowledge of lifestyle modifications among hypertensive patients?
1.5 Research Hypothesis
In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.
Hypothesis One
- H0: Participation in a structured training program do not significantly improve the knowledge and practice of lifestyle modifications among hypertensive patients attending out-patient clinics.
- H1: Participation in a structured training program significantly improves the knowledge and practice of lifestyle modifications among hypertensive patients attending out-patient clinics.
Hypothesis Two
- H0: There is no significant improvement in the knowledge and practice of lifestyle modifications among hypertensive patients attending out-patient clinics following participation in a structured training program
- H1: There is a significant improvement in the knowledge and practice of lifestyle modifications among hypertensive patients attending out-patient clinics following participation in a structured training program
1.6 Significance of Study
The outcome realized from the research findings will be significant to the following stakeholders:
- Healthcare Providers: Improving patient adherence to lifestyle modifications can lead to better blood pressure control, reduced healthcare costs associated with hypertension-related complications, and improved patient outcomes.
- Patients: Enhanced knowledge and practice of lifestyle modifications can empower patients to actively participate in managing their hypertension, potentially leading to better quality of life and reduced risk of cardiovascular events.
- Healthcare Systems: Effective training programs can contribute to more efficient use of healthcare resources, reduced hospital admissions related to uncontrolled hypertension, and overall improvement in population health outcomes.
- Policy Makers: Evidence of the effectiveness of training programs can inform healthcare policies aimed at integrating such interventions into standard hypertension management protocols, potentially reducing long-term healthcare costs and improving public health.

1.7 Scope of Study
The scope of the research is focused on the effect of a training programme on knowledge and practice of lifestyle modification among hypertensive patients attending out-patient clinics using Warri Central Hospital in Delta State as a case study.
1.8 Limitations of the Study
During the course of this study, there were some problems encountered which stood as limitations to the research work. Some of the limitations include:
- Time Constraint: The time frame given to accomplish this project was very short due to school academic calendar and it was carried out under pressure which made the researcher not to implement some necessary features.
- Financial Constraint: Insufficient fund tends to impede the efficiency of the researcher in sourcing for the relevant materials, literature or information and in the process of data collection (internet, questionnaire and interview).
- Initial Cooperation Delay from Respondents: A particular limitation of this work came as a result of the respondent refusal to offer their cooperation at the initial time they were contacted. This contributed in making the success of this research study difficult.
1.9 Definition of Terms
Hypertension:
Hypertension, or high blood pressure, is defined as a systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥90 mmHg. It is a major risk factor for cardiovascular diseases such as heart attacks and strokes (WHO, 2019).
Lifestyle Modifications:
Lifestyle modifications refer to changes in diet, physical activity, weight management, and reduction in alcohol and tobacco use aimed at improving health outcomes, particularly in managing chronic conditions like hypertension.
Training Programme:
A structured training programme in this context refers to an educational intervention designed to provide hypertensive patients with knowledge, skills, and support to effectively implement lifestyle modifications.
Out-Patient Clinics:
Out-patient clinics are healthcare facilities where patients receive medical care without being admitted to a hospital overnight, typically for routine check-ups, consultations, and management of chronic conditions.
