1.1 Introduction
Urinary tract infection (UTI) is defined as the presence and multiplication of pathogenic microorganisms in any part of the urinary system, including the kidneys, ureters, bladder, and urethra, which leads to clinical symptoms such as dysuria, frequency, urgency, and sometimes systemic manifestations like fever (Flores Mireles et al., 2015). UTIs are one of the most common bacterial infections worldwide, affecting people of all ages and sexes, with a higher prevalence among females due to anatomical and physiological factors (Gupta et al., 2011). The infection is often caused by gram-negative bacteria, particularly Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Pseudomonas aeruginosa, which are responsible for the majority of community-acquired and hospital-acquired UTIs (Hooton, 2012).
As a prelude to other parts of this study, this chapter will discuss the background upon which this study was initiated, the statement of problems that led to this study, the Aim and Objectives of the study. Others are significance of the study, scope of work, research questions, limitation of the study and definition of terms.
1.2 Background of Study
Urinary tract infections (UTIs) are among the most common bacterial infections affecting humans, with a substantial proportion caused by gram-negative bacteria. According to Flores Mireles et al. (2015), UTIs are characterized by the presence and proliferation of pathogenic microorganisms in the urinary system, resulting in clinical symptoms such as dysuria, increased urinary frequency, urgency, and, in severe cases, systemic manifestations like fever and malaise. These infections are particularly prevalent among young adults and women due to anatomical, hormonal, and behavioral factors (Gupta et al., 2011).
Several studies have highlighted the predominance of gram-negative bacteria in UTIs. Hooton (2012) reported that Escherichia coli is responsible for approximately 70–90% of community-acquired UTIs, followed by Klebsiella pneumoniae, Proteus mirabilis, and Pseudomonas aeruginosa. It is asserted that these pathogens possess virulence factors such as adhesins, toxins, and biofilm-forming ability that facilitate colonization of the urinary tract and contribute to infection severity (Wiles et al., 2008). On the other hand, Pitout and Laupland (2008) stated that the emergence of multidrug-resistant gram-negative bacteria has significantly complicated UTI management globally, making it critical to understand local susceptibility patterns for effective treatment.
The spread of antibiotic-resistant bacteria has been attributed to overuse and misuse of antimicrobials. Schito (2001) affirmed that resistance among urinary pathogens varies geographically and temporally, necessitating regular surveillance to guide empirical therapy. In university settings, students are at increased risk due to shared accommodations, close interpersonal contact, and occasional neglect of personal hygiene practices, which favor the transmission of uropathogens (Onoh et al., 2013). It is contended that asymptomatic bacteriuria in such populations may go undetected, potentially leading to complications like pyelonephritis or recurrent infections. This study is set against the backdrop that this study seeks to investigate the prevalence and antimicrobial susceptibility of gram-negative bacteria in the urine of students.
1.3 Statement of Problems
Investigation revealed that the burden of urinary tract infections (UTIs) among young adults, particularly university students, is increasingly becoming a public health concern because these infections are often associated with gram negative bacteria that exhibit evolving resistance to commonly prescribed antibiotics. In many tertiary institutions, students live in close quarters, share facilities, and sometimes engage in behaviors that predispose them to UTIs, yet there is limited localized evidence on the specific bacteria involved and how these organisms respond to current antimicrobial therapies (Flores Mireles et al., 2015).
The prevalence of gram negative bacteria in the urine of Caritas University students is not well documented, leaving health practitioners and campus health services without clear epidemiological data to inform diagnosis and treatment. Without current prevalence data, there is a risk that asymptomatic bacteriuria or symptomatic UTIs may be misdiagnosed or undertreated, which is particularly concerning given the global rise of multidrug resistant organisms (Gupta et al., 2011).
Furthermore, the absence of context specific antimicrobial susceptibility data is not only a clinical issue but also has broader implications for antimicrobial stewardship efforts within and beyond the university community. If local resistance trends are unknown, there is limited capacity to develop evidence based treatment guidelines or to educate students and healthcare providers about responsible antibiotic use (Hooton et al., 2010). It is against this backdrop that this study seeks to determine the prevalence of gram-negative bacteria in the urine of Caritas University students and assess their antimicrobial susceptibility patterns.
1.4 Aim and Objectives of Study
The aim of this study is to investigate the prevalence and antimicrobial susceptibility of gram-negative bacteria in the urine of Caritas University students.
In achieving this aim, the following specific objectives were laid out as follows:
- To determine the prevalence of gram-negative bacteria in the urine of Caritas University students.
- To identify the specific types of gram-negative bacteria present in the urine samples.
- To assess the antimicrobial susceptibility patterns of the isolated gram-negative bacteria.
- To evaluate the potential risk factors associated with urinary tract infections among the students.
1.5 Research Questions
The study came up with research questions so as to be able to ascertain the above stated objectives. The specific research questions for the study are stated below as follows:
- What is the prevalence of gram-negative bacteria in the urine of Caritas University students?
- Which types of gram-negative bacteria are present in the urine samples?
- What are the antimicrobial susceptibility patterns of the isolated gram-negative bacteria?
- What are the risk factors contributing to urinary tract infections among the students?
1.6 Research Hypothesis
In order to pursue the objective of this study, the following generalized statements have been designed to guide and aids in obtaining the result for the experiment to be conducted. For this work, the null hypothesis will be represented with H0 while the alternative hypothesis will be represented with hypothesis H1.
- H0: There is no significant prevalence of gram-negative bacteria in the urine of Caritas University students, and the bacteria are not resistant to commonly used antibiotics.
- H1: There is a significant prevalence of gram-negative bacteria in the urine of Caritas University students, and the bacteria exhibit resistance to commonly used antibiotics.
1.7 Significance of Study
It is believed that at the completion of the study, the research will provide accurate information on the prevalence and antimicrobial susceptibility of gram-negative bacteria among Caritas University students. Also, the study will increase awareness of urinary tract infections and encourage preventive health practices.
Furthermore, the study will provide updated information on local antimicrobial susceptibility patterns to aid in prescription decisions. In addition, the research will inform policies on infection control and antibiotic stewardship within tertiary institutions.
Lastly, this knowledge will assist health authorities in designing effective treatment protocols, improving student health outcomes, and preventing the spread of antibiotic-resistant bacteria.
1.8 Scope of Study
The study will focus on Caritas University students in Enugu State, Nigeria. It will involve the collection and analysis of urine samples to determine the prevalence and antimicrobial susceptibility of gram-negative bacteria. The study is limited to students currently enrolled in the university and does not extend to staff, surrounding communities, or other tertiary institutions.
1.9 Limitations of the Study
During the course of this study, there were some problems encountered which stood as limitations to the research work. Some of the limitations include:
- Time Constraint: The time frame given to accomplish this project was very short due to school academic calendar and it was carried out under pressure which made the researcher not to implement some necessary features.
- Financial Constraint: Insufficient fund tends to impede the efficiency of the researcher in sourcing for the relevant materials, literature or information and in the process of data collection (internet, questionnaire and interview).
- Initial Cooperation Delay from Respondents: A particular limitation of this work came as a result of the respondent refusal to offer their cooperation at the initial time they were contacted. This contributed in making the success of this research study difficult.
1.10 Definition of Terms
Urinary Tract Infection (UTI): According to Flores-Mireles et al. (2015), a UTI is an infection of any part of the urinary system caused by pathogenic microorganisms, resulting in symptoms such as dysuria, frequency, and urgency.
Gram-negative Bacteria: These are bacteria that do not retain the crystal violet stain in the Gram staining procedure and include pathogens such as Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa, which are commonly associated with UTIs (Hooton, 2012).
Antimicrobial Susceptibility: The sensitivity of bacteria to antibiotics, which determines the effectiveness of the drug against specific bacterial isolates (Pitout & Laupland, 2008).
Prevalence: The proportion of individuals in a population who have a particular infection at a given point in time (Gupta et al., 2011).
Multidrug Resistance: The ability of bacteria to resist the effects of multiple antimicrobial agents, making infections harder to treat (Schito, 2001).
