1.0 Introduction
1.1 Background of Study
Hypertension, also known as high blood pressure, is a major public health issue and one of the leading causes of cardiovascular diseases worldwide. It is a chronic medical condition characterized by persistently elevated arterial blood pressure, which increases the risk of stroke, heart disease, and kidney failure (World Health Organization [WHO], 2023). The condition is particularly prevalent among elderly individuals due to age-related physiological changes, including arterial stiffness and reduced vascular elasticity (James et al., 2021). These changes contribute to the increased incidence and severity of hypertension in older adults.
The global burden of hypertension is alarming, with over one billion people affected, and the prevalence is significantly higher in elderly populations (Centers for Disease Control and Prevention [CDC], 2023). In many developing countries, including Nigeria, the prevalence of hypertension among the elderly is rising due to urbanization, sedentary lifestyles, and dietary changes (Okafor & Adeyemi, 2022). Unfortunately, awareness and control rates remain low, resulting in a high incidence of hypertension-related complications such as stroke, heart failure, and chronic kidney disease (Brown et al., 2022). Uncontrolled hypertension in elderly individuals presents unique challenges in management, as it often coexists with other chronic conditions such as diabetes, arthritis, and cognitive impairment (Smith & Johnson, 2020). Additionally, poor adherence to medication, limited healthcare access, and socio-economic factors contribute to poor hypertension control among this age group (Williams et al., 2022). While pharmacological treatments play a crucial role in blood pressure management, non-pharmacological interventions such as dietary modifications, physical activity, and stress management are equally important in reducing hypertension-related complications (American Heart Association [AHA], 2022).
Hypertension is a significant public health concern, especially among elderly individuals. Aging is associated with physiological changes that affect the elasticity of blood vessels, leading to increased blood pressure levels (James et al., 2021). The prevalence of hypertension among older adults is notably high, with studies indicating that over 60% of individuals aged 60 and above suffer from the condition (Centers for Disease Control and Prevention [CDC], 2023). The progressive nature of hypertension makes early detection, proper management, and adherence to treatment essential in preventing severe complications such as stroke, heart failure, and renal dysfunction (Smith & Johnson, 2020).

Therefore, this study seeks to explore the causes, complications, and management of hypertension among elderly individuals.
1.2 Statement of Problems
Investigation revealed that many elderly individuals are either unaware of their hypertensive status or do not receive proper medical attention until complications arise. Inadequate health education, limited access to healthcare services, and financial constraints make it difficult for many older adults to seek early diagnosis and consistent treatment (Brown et al., 2022). Even when treatment is available, adherence to antihypertensive medication is a challenge due to forgetfulness, lack of understanding of the condition, and concerns about medication side effects (Williams et al., 2022).
Furthermore, the impact of lifestyle factors such as poor diet, physical inactivity, stress, and excessive salt intake is another critical issue. Many elderly individuals struggle to adopt and maintain healthier lifestyles due to mobility limitations, cultural food preferences, and the absence of strong social support systems (James et al., 2021). As a result, non-pharmacological interventions that are known to be effective in managing hypertension are often underutilized.
In many developing countries, the burden of hypertension among the elderly is worsened by weak healthcare systems, inadequate funding for geriatric care, and a lack of trained medical personnel specializing in elderly health (Okafor & Adeyemi, 2022). The absence of structured hypertension screening programs means that many elderly individuals remain undiagnosed until they experience severe complications. The psychological impact of living with a chronic illness also contributes to poor management, as depression and anxiety often reduce the motivation to seek care and adhere to treatment (Anderson & Lee, 2022). It is against the backdrop that this study seeks to address these problems by identifying challenges in diagnosis, treatment, and prevention strategies to improve health outcomes and quality of life.
1.3 Aim and Objectives of Study
The aim of this study is to examine the effects, causes, complications, and management of hypertension among elderly individuals.
The specific objectives of the study are as follows:
- To assess the effectiveness of current management strategies, including both pharmacological and non-pharmacological interventions.
- To evaluate the role of lifestyle modifications, healthcare accessibility, and social support in hypertension management.
- To examine the effects of hypertension on the overall health and well-being of elderly people.
- To analyze the common complications associated with uncontrolled hypertension in older adults.
- To explore factors influencing adherence to hypertension treatment among elderly individuals.
- To identify the primary causes of hypertension among elderly individuals.
- To provide recommendations for improving hypertension awareness, prevention, and control among elderly populations.
1.4 Research Questions
Based on the stated objectives, the following research questions will guide this study on the effects of Public-Private Partnerships (PPPs) on service delivery:
- What are the primary causes of hypertension among elderly individuals?
- How does hypertension affect the overall health and well-being of elderly people?
- What are the common complications associated with uncontrolled hypertension in older adults?
- How effective are current management strategies, including pharmacological and non-pharmacological interventions, in controlling hypertension among the elderly?
- What factors influence adherence to hypertension treatment among elderly individuals?
- What role do lifestyle modifications, healthcare accessibility, and social support play in the management of hypertension among elderly populations?
- What recommendations can be made to improve hypertension awareness, prevention, and control among elderly individuals?
1.5 Research Hypotheses
Based on the stated objectives, the following hypotheses have been formulated to guide this study:
Hypothesis One
- H0: There is no significant relationship between hypertension management strategies and the reduction of complications among elderly individuals
- H1: There is a significant relationship between hypertension management strategies and the reduction of complications among elderly individuals
Hypothesis Two
- H0: Hypertension among elderly individuals is not significantly influenced by lifestyle factors such as diet, physical activity, and stress
- H1: Hypertension among elderly individuals is significantly influenced by lifestyle factors such as diet, physical activity, and stress
1.6 Significance of Study
The outcome of this research will help healthcare providers recognize the challenges elderly individuals face in managing hypertension, including issues related to medication adherence, lifestyle modifications, and access to healthcare services. Also, public health policymakers will benefit from the research study by gaining evidence-based recommendations for implementing programs aimed at reducing hypertension-related complications in aging populations.
Furthermore, this research will serve as a foundation for future studies on elderly hypertension, paving the way for further exploration of innovative management techniques, alternative treatments, and policy-driven solutions to enhance the quality of life for older adults affected by hypertension.
1.7 Scope of Study
This study will focus on the effect, causes, complications, and management of hypertension among elderly individuals in Lagos State, Nigeria. The scope will cover a detailed examination of the primary causes of hypertension in the elderly, including lifestyle factors, genetics, and other socio-economic variables prevalent in Lagos State.
1.8 Limitations of the Study
A study of this nature is bound to experience certain problems as such the constraints imposed on the research include:
- Response Bias: There is a potential for response bias, especially from elderly individuals who may not fully understand the details of their health conditions or may be reluctant to disclose sensitive information related to their treatment or lifestyle choices.
- Data Collection Challenges: Access to elderly participants may be constrained by logistical issues, such as transportation, mobility challenges, and communication barriers. Furthermore, obtaining accurate medical records and healthcare data may be difficult due to incomplete or inconsistent documentation, particularly in public healthcare facilities.
- Time Constraints: The research is bound by time limitations, which may affect the depth of the study, particularly in terms of long-term follow-up or the assessment of long-term treatment outcomes in elderly patients with hypertension.
- Cost: The research would have extended the survey to other area at the empirical level, but limitation as included cost of transportation to the source of material and the cost of time setting of the already completed work.
1.9 Definition of Terms
Hypertension:
Hypertension, commonly known as high blood pressure, is a medical condition characterized by consistently elevated blood pressure levels above 140/90 mmHg (World Health Organization [WHO], 2020). It is often referred to as the "silent killer" because it may not present noticeable symptoms but can lead to serious health complications such as heart disease, stroke, and kidney damage if left unmanaged.
Elderly People:
Elderly people, in the context of this study, refer to individuals who are aged 60 years and above (United Nations, 2019). The elderly population is particularly vulnerable to various health conditions, including hypertension, due to the physiological changes associated with aging, such as a decrease in the elasticity of blood vessels and an increase in vascular resistance.
Complications of Hypertension:
Hypertension-related complications refer to the severe health problems that arise as a result of uncontrolled high blood pressure. These include conditions such as stroke, heart failure, kidney disease, and cognitive decline (American Heart Association [AHA], 2020). These complications can significantly impair the quality of life and lead to disability or death, particularly among elderly individuals.
Management of Hypertension:
The management of hypertension involves various strategies aimed at controlling blood pressure levels and preventing complications. These may include lifestyle modifications (such as dietary changes, physical activity, and stress management), pharmacological treatments (such as antihypertensive medications), and regular monitoring by healthcare providers (National Institute for Health and Care Excellence [NICE], 2019).
Pharmacological Treatment:
Pharmacological treatment refers to the use of medications to manage hypertension. Common antihypertensive drugs include diuretics, beta-blockers, calcium channel blockers, and ACE inhibitors. These medications help reduce blood pressure by either relaxing blood vessels, reducing the heart's workload, or eliminating excess fluid from the body (Mayo Clinic, 2021).
Non-Pharmacological Treatment:
Non-pharmacological treatment includes lifestyle interventions that do not involve medications. These strategies focus on improving health through diet, physical activity, weight management, stress reduction, and reducing alcohol and tobacco consumption (American College of Cardiology [ACC], 2021).
